Meet the Doctors
Jessica Min, OD
Dr. Jessica Min grew up in the northern suburbs of Chicago. She received her B.S. in Molecular and Cellular Biology at the University of Illinois- Urbana, Champaign. She then earned her Doctor of Optometry degree and graduated Cum Laude from the Illinois College of Optometry. After practicing for a couple years in various clinical settings, she realized she had a true passion for the pediatric population and went on to complete a post-graduate residency in Pediatrics and Binocular Vision Disorders at the Illinois College of Optometry. She is always striving to provide the best eye care for her patients and is a dedicated lifelong learner to be a well-rounded optometric physician.
Shirin Patel, OD
Dr. Shirin Patel graduated from the University of Illinois at Chicago where he received his Bachelor of Science in Molecular Biology. Following his undergraduate studies, Dr. Patel went on to the Illinois College of Optometry where he earned his Doctorate of Optometry. He completed clinical rotations in various specialties including vision therapy at the Illinois Eye Institute, ocular disease at the North Chicago VA Hospital, and primary care at a private practice in Las Vegas. His specialty is in primary care optometry as well as treatment and management of ocular disease.

Comprehensive Eye Exam
A comprehensive eye exam is a thorough assessment of the eyes to rule out eye disease and vision problems. This exam provides the patient with a glasses prescription and a wide-field retinal image. However, if any medical abnormalities are found a dilated exam will be recommended.
Contact Lens Exam
A contact lens evaluation includes a comprehensive eye exam to review the health of your eyes and to provide the patient with both a glasses and a contact lens prescription. Contact lens fitting fee varies from a simple to complex fit.
All first time contact lens wearers must complete a contact lens training class
Contact Lens Fitting Only
In situations where the patient has already had a comprehensive eye exam within the past 3 months at a different office and has an updated glasses prescription you are eligible for this option. The patient must provide proof of the updated glasses prescription in order to do the contact lens fitting only and receive a contact lens prescription.
Medical Office Visit
A medical office visit focuses on eye conditions such as eye pain, dry eyes, itchy eyes, red eyes, flashes/floaters, foreign bodies, lid infections and cataracts. Depending on the complexity of the condition a medical prescription may be provided, additional testing might be recommended or a referral to a specialist.

Do I have to be a member to see you?
No, we are an independent practice located inside the Pleasant Prairie Costco. Our mission is to provide high quality eye care at an affordable price for both members/non-members.
Do I have to be a member to purchase from the Costco Optical?
Yes, you need to have a membership to purchase glasses and contacts.
Do you take insurance?
We accept VSP, Spectera/United Healthcare Vision and Costco Employee EyeMed vision insurance. For all other insurances, we can provide you with an itemized receipt and assist you in the reimbursement process for your specific insurance plan. The Costco Optical has its own accepted list of insurances.
